Revision Rhinoplasty Before & After: Crooked Nose & Nasal Obstruction Correction with MTF Cartilage Grafts

Our beautiful patient is seen before, and one year after complex revision rhinoplasty surgery utilizing MTF fresh frozen cadaver rib cartilage. Her initial rhinoplasty surgery left her with a crooked nasal appearance, a severely deviated nasal septum, weak external nasal valves, and an overall distracting nasal appearance. Since she had a history of chronic nasal allergy, her nasal obstruction was worsened by chronic turbinate enlargement. In our experience, lateral crural extension grafts should be placed in most patients with nasal obstruction at the time of revision rhinoplasty surgery to improve nasal breathing, and to prevent worsening of somebody's nasal airway. Since she did not have an adequate amount of her own nasal septal cartilage remaining, that is the reason why we purchased a sheet of Profile MTF cartilage which worked very well, and has stood the test of time. We utilized the MTF cartilage to place a large right sided spreader graft, and to support her external nasal valves. We utilized her own cartilage for a columella strut graft, and for an extended shield graft, for tip definition and support. At her revision rhinoplasty anniversary, you can see a less distracting, and more feminine nasal appearance. We are also pleased to report that she is breathing much better. When we examine her, we can feel the MTF grafts in place and as they were at the time of surgery. In our experience, we have not seen resorption, and I believe this is a better alternative to autologous rib cartilage, since there is no donor site morbidity, and since these grafts are versatile and decrease surgical time.
