Rhinoplasty Before & After: Results After Three Years

We were thrilled to see our beautiful 24-year-old patient today, 3 YEARS after functional and cosmetic rhinoplasty surgery, and lower eyelid blepharoplasty to remove her congenital under-eye bags. You can see in her preoperative photos that there is a cleft between her tip cartilages, which is indicative of thin nasal skin. Generally, those with thin nasal skin are more likely to see imperfections or irregularities after rhinoplasty surgery, so skillful and well-planned surgery is most important.

When she came to visit us, she did not like her bulbous and over-projected nasal tip appearance. You can see that her nose was too long, that her wide tip stuck out too far from her face, and that this was associated with a dorsal hump. Since she had thin skin and was noted to have collapse of her external nasal valves with inspiration, her functional surgery consisted of correcting her deviated nasal septum and reinforcing her external nasal valves with lateral crural extension grafts. These cartilage grafts were repurposed from her deviated nasal septal cartilage that was causing obstruction.

From a cosmetic standpoint, our goal was to create narrow, straight bridge lines that would descend to a circular, lifted, and deprojected nasal tip appearance, along with lowering her dorsal hump for a less distracting and more feminine nasal appearance. Whenever considering rhinoplasty surgery, it is very important, in our opinion, to ensure that your prospective surgeon posts long-term follow-up results. In most cases, it takes 12 months or longer for someone to realize their final result. Additionally, you can see a natural-looking and rejuvenated under-eye appearance without her under-eye fat pads that were previously distracting preoperatively.

We also included her computer imaging. The goals of computer imaging are not to create a replica of a post-surgical result; rather, the goals are to ensure that patients have realistic expectations before undergoing surgery. We always use computer imaging in a conservative fashion, so our actual results may exceed someone’s expectations.

A 44-year-old gentleman is seen well over one year following both functional and cosmetic rhinoplasty surgery. Preoperatively, he did not like that his nose projected too far in profile, and he was unhappy with his profile bump. Additionally, he experienced nasal obstruction on both sides due to a severely deviated nasal septum and congenital weakness of both his internal and external nasal valves. In his 14-month postoperative photos, you can see a less projected nasal profile that blends naturally and maintains a masculine appearance, without signs of previous rhinoplasty surgery. His functional repair required septoplasty, lateral crural strut grafts, and bilateral spreader grafts. We are so grateful that he has allowed us to share his photographs, and his nasal airway is now breathing perfectly.

Our beautiful 28-year-old patient is seen six and a half months following rhinoplasty surgery at our practice. She was dissatisfied with the wide appearance of her nose, which was associated with a droopy tip and a dorsal hump. As seen here, we achieved a natural-looking nasal appearance through tip rotation, tip narrowing, nostril narrowing, nasal narrowing, and hump removal, resulting in a less distracting and more feminine profile. Her alarplasty scars are well-concealed and limited to the base of her nose.

Her after photos were taken 6 months following her rhinoplasty surgery. The goals of rhinoplasty surgery are to help a person's nose blend in naturally, not to turn it into the centerpiece of the face. We can now see a more feminine and less distracting nasal appearance, without any stigmata of previous nasal surgery. We are extremely grateful that she has allowed us to share her photos.
